This report will give you information regarding a child's Medical Condition/s, Immunisation Status, Allergies or any Restrictions relating to the Child.
This report is available in either a Excel file or PDF.
TABLE OF CONTENTS
Building the report
To locate the report select Analytics from the main menu, then select Exports from the drop down options.
Select Child Health & Wellness Report (by child)
Select Run Report
Complete the details in the drop down menu - Centre, Class, Start/End Date and Format(s)
Select Generate Report
If you run the report in Excel file you can then modify the report.
Modify the report
The report will then build as an excel spreadsheet. You can format the report to utilise this for various information. Each heading will then have a dropdown that you can filter each heading by:

The report will include:
- The Category
- Condition Type
- Condition Name
- Symptoms
- Care Instructions
